A Team Approach

 

JADP provides full-time medical over site by registered nurses. This team has years of experience helping aging individuals and those with memory issues.  This team administers medications, monitors health status and helps coordinate care with your health care providers.

COMMUNICATING WITH PHYSICIANS

We are now working even more closely with the physicians who care for our participants.  Every six months we send a Participant Progress Update to participant physicians. The report indicates our understanding of medications, health and behavioral issues that relate to the participant's care. This helps both the doctor and the Center staff address the changing health care needs of our participants and encourages close communications between health professionals and families.

REFERRALS & RECOMMENDATIONS

In addition to providing adult day programming services, The Johnson center offers caregivers and family members information and referral recommendations to meet additional health needs including case management services. Contact us if there is anything that we can be of assistance with!
