Case Management
In addition to providing adult day programming services, The Johnson center offers caregivers and family members information and referral recommendations to meet additional health needs including case management services. Our social worker, Mark Grimm (see alphabetical listing on staff page) , is available to counsel families by appointment. With more than 30 years experience working with elders, particularly those with dementia, and their families, Mark is a respected professional in the health care community.
Both Jewish Family Services (www.jewishfamilyservices.org) and Lutheran Family Services (www.lfsco.org) in the Denver area are good sources for case management services.
